1、Authentication of the certificate Incorporation (CI ) proved by China embassy (Two original).外国公司开业证明需由当地公正机关或律师做公证后，并由当地大(领事)使馆认证（原件两份）。

2、The bank financial credit certificate of investors.外国企业银行资信证明原件。

3、One copy of identification card (or passport) of the legal representative person & supervisor of the junior enterprise.  董事、监事的身份证或护照复印件。（外资公司可以不设立董事会，需设立一名执行董事，并提供执行董事委派书及身份证明复印件）

4、Business scope.  经营范围

5、 Legal representative person& directors ‘s resume (include address,telephone etc.) , one copy of identification card, two legal representative person's photos (size 2 inch). 法人代表简历及董事简历包括电话、住址等。（拟设上海公司法人代表任职文件，身份证明复印件，简历，一寸或两寸彩色照片两张）

6、 Office Lease Agreement which should be in its original contract . 房产证复印件两份，复印件加盖房屋业主公章。

7、lease agreement In Shanghai Office & certificate of house property copy (Three original). 上海办公室租赁合同原件三份。

* Lease agreement must be put on records in real estate Bureau.
